Your account
- Provide accurate, complete, and current information when creating an account
- Maintain the confidentiality of your login credentials
- You are responsible for all activities under your account
- Notify us immediately of any unauthorized use
Acceptable use
- Use the Service only for lawful purposes related to community health programs, case coordination, and grant reporting
- No violation of applicable laws or regulations
- No malware, no unauthorized access attempts, no reverse engineering
No Protected Health Information (PHI)
CH360 is a program management and reporting platform for community-based organizations. It is not designed, intended, or authorized to receive, store, transmit, or process Protected Health Information (PHI) as defined by HIPAA. CH360 does not offer Business Associate Agreements (BAAs).
- You agree not to enter diagnoses, medications, treatment plans, clinical narratives, or any other PHI into the Service
- You are solely responsible for ensuring data you enter does not constitute PHI under HIPAA or applicable state law
- If PHI is entered in error, notify us promptly so we can assist with removal
