Explore the value around your hub

Make funding accountability part of daily work.

Connect grant lifecycle management and reimbursement readiness to the services your network delivers. Build clearer evidence for proposals, reporting, and renewals.

Your network delivers the work.
Make that work easier to prove and sustain.

CH360 connects frontline operations to the evidence behind your network’s next funding request, contract conversation, and renewal.

One platform. The whole service lifecycle.

Fund the services.
Sustain the network
that makes them possible.

Care hubs carry the work of coordination alongside the needs of their CBOs. CH360 brings service delivery and funding accountability into a connected operating and reimbursement platform.

01

Get Funded: show the case for investment

Use documented services and outcomes to explain your network’s reach, community needs, and contribution.

02

Get Paid: connect services to billing

Use documented services to prepare claims and invoices for staff review. Confirm eligibility, authorization and payer requirements before submission.

03

Keep Serving: give teams time for people

Reuse service information across workflows and reports so CBOs spend less time rebuilding evidence and more time serving people.

Customizable screenings, intakes & forms

The right information.
At the first interaction.

Your funders ask specific questions. Your CBOs serve different communities. Shape data collection around both.

Build questions and answer choices for screenings, program intakes, and follow-up forms. Plan shared network requirements alongside the details each program needs.

Preparing for Community Information Exchange

Your CIE journey
starts with a
prepared network.

Help your CBOs establish consistent records and usable workflows today. Use that foundation to define what your future Community Information Exchange needs to share.

CH360 can support the operational preparation for that journey. Your network leads the governance, agreements, and decisions about appropriate sharing.

Discuss your CIE goals
01 / SHARED LANGUAGE

Agree on what to capture

Define common information, service descriptions, and outcomes across participating CBOs.

02 / DAILY WORKFLOWS

Make participation workable

Plan documentation and follow-up that fit frontline service delivery and staff capacity.

03 / NETWORK GOVERNANCE

Set sharing expectations

Identify consent requirements, access boundaries, data ownership, and partner responsibilities.

04 / TECHNICAL PLANNING

Define the future connection

Document the data and system requirements that a future CIE integration would need to address.

Where we are today: CH360 does not currently interoperate with outside CIE, EHR, or referral systems. CIE preparation is an operational foundation. External connections require separate technical assessment, agreements, and development.

Answers for hub leaders

A clearer picture.
Before you commit.

Your network’s visibility, permissions, and reporting scope should be concrete before rollout.

Walk through your workflow
Can our hub see across participating CBOs?

We’ll define the oversight your hub needs: aggregate results, organization-level views, or access to specific records. Available network views and permissions must be confirmed in your deployment scope. Hub membership alone does not grant access to every CBO record.

Can each CBO keep its own forms and workflows?

CH360’s customizable forms support questions and answer choices for different programs. Bring examples of your shared requirements and local forms so we can confirm the available configuration and any additional needs.

Can one record support grants and reimbursement?

CH360 connects service documentation with grant lifecycle management, reporting, and claims preparation. Staff review the information before use. Reimbursement depends on the applicable eligibility and payer requirements, and funding or payment is not guaranteed.

Will we know whether a referral led to a service?

That is an important network requirement. We’ll demonstrate the available partner coordination workflow and confirm how responsibility, follow-up, and service delivery are recorded for your deployment.

Is CH360 already connected to our CIE or EHR?

No. CH360 does not currently interoperate with outside CIE, EHR, or referral systems. It can help establish the operating foundation for future CIE participation. Any external exchange requires separately scoped technical work and agreements.

What about security, consent, and data ownership?

A BAA is required before use involving protected health information. We’ll review security documentation and define access, applicable consent requirements, ownership, exports, and offboarding arrangements for your network. Specific controls should be demonstrated before rollout.

Can the hub sponsor CBO access and roll out in phases?

We can discuss network sponsorship and a phased deployment around a defined group or program. Your proposal should specify setup, training, support, ongoing cost, and the agreed capabilities before you expand.

Does CH360 send our reports automatically?

CH360 supports end-user report generation and staff review. Scheduled exports or automatic delivery should be confirmed separately in your scope rather than assumed.
